QUALIFICATIONS FOR MEMBERSHIP TO PSOPRS
Membership in the Society shall be limited to a licensed ophthalmologist who fulfills the following:
- Completed Post-Graduate Fellowship in Ophthalmic Plastic and Reconstructive Surgery for a minimum cumulative period of 12 months.
- Committed to indulge in the practice, research and teaching of Ophthalmic Plastic and Reconstructive Surgery
- Has good moral character, reputation, ethical and professional standing
- A diplomate of the Philippine Board of Ophthalmology
- A member in good standing of the Philippine Academy of Ophthalmology
- Who meet such other qualifications as described in the By-laws of the Society
REQUIREMENTS FOR APPLICATION*

dcu9Yq5g0YEPRKby2 ) ** - Passport size or 2x2 colored picture (attached to Letter of intent)
- Letter of Intent addressed to the current PSOPRS President (Raoul Paolo D. Henson,MD)
- Updated Curriculum Vitae
- Letter of Recommendation from 2 Fellow members in good standing
- Application Processing Fee: php 1,500.00
- Copy of the following documents:
- Medical School Diploma
- Updated PRC ID
- Certificate of Residency in Ophthalmology
- Certified True Copy of Certificate of Fellowship
- The original certificate may be presented and a copy may be submitted after being certified by the current Chairman of the Membership Committee
- PBO Certificate as Diplomate in Ophthalmology
- PAO Certificate of Membership and Good Standing
